Your Prescriptions

To obtain a prescription, please bring, fax or e-mail your request to the surgery.  Please use the counterfoil of your previous prescription and tick the items required.  If

You have misplaced this, please write your name, date of birth, name, strength and quantity of the items required on apiece of paper and bring, post or fax to

The surgery.  If a prescription request arrives at the surgery before 11 am it will be ready 48 hours later.  If you wish to collect your prescription from the surgery or the chemist please state clearly on the request form.

Telephone requests will not be accepted unless a patient is totally housebound and has made prior arrangements with the surgery.
